Staff are available to help you with referrals and trip planning Monday – Friday from 7:00 am – 7:00 pm.  You can call after 7:00 pm and on the weekends and leave a message which will be returned promptly during normal call center hours.

In Tompkins County call 2-1-1 or 1-877-211-8667 toll-free.

2-1-1 is available 24 hours a day, 365 days a year to help you connect to the resources you need including information about transportation services that are available to help you get to your medical appointments.

Home Health Monitoring

Area health care systems offer options to monitor patients’ blood pressure, heart rate, temperature, and other vital signs remotely.  Providers use telehealth monitors to transmit information from the patients’ homes to nurses, who review and follow up directly with patients.

The Southern Tier Mobile Integration Team (MIT), based at the Elmira Psychiatric Center and the Greater Binghamton Health Center, brings together a team of mental health professionals and paraprofessionals to provide services for children and adults in the home or other community settings.  The MIT serves a 15-county region including counties in the Southern Tier.

In Tompkins County, the Collaborative Solutions Network CARE Teams (Collaborate, Align, Respond, Engage) assemble an interdisciplinary team to work with individuals facing social and emotional challenges.  By bringing together an interdisciplinary team in a convenient location for the individual, transportation needs and time spent scheduling and attending meetings is reduced.

Center of Treatment Innovation operated by the Addiction Center of Broome County and CASA-Trinity offer mobile services for individuals with substance use disorders.
