HealthlinkNY Community Network: A Healthcare Catalyst in the Hudson Valley
The HealthlinkNY Community Network works with the community to address population health challenges and to reduce disparities in the Hudson Valley of New York. Building on the successful work of our stakeholders, we provide a neutral forum in which to identify, share, and disseminate best practices and strategies to improve health outcomes in the region.
We have 40 partners representing diverse sectors such as healthcare, education, housing, behavioral health, labor, economic development, government, consumer advocacy, insurance, and public health. Together, we’re improving health outcomes in all of the seven counties in the Hudson Valley region: Dutchess, Orange, Putnam, Rockland, Sullivan, Ulster, and Westchester.
Thanks to a series of training events, we are successfully promoting population health improvement in the important areas of health equity, mental health, addiction, and diabetes.
