The HealthlinkNY Patient Portal will be unavailable beginning March 31, 2018, due to reductions in funding from the New York State Department of Health. We apologize for this inconvenience.
We strongly suggest that you download your medical record from the site before March 31, 2018, as that information is still an invaluable tool for you. You can download a copy of that record by completing the following steps: 
  • Log into your HealthlinkNY Patient Portal account here
  • Click the "Data" tab found on the left-hand side of your HealthlinkNY Patient Portal account homepage 
  • Click the "Download" icon found on the right-hand side of the "Data" page 
  • From there, select the type of summary you want to download that best fits your needs 
  • Once you select a summary to download, click the "PDF" button at the bottom of the Download page and your medical record will be downloaded to your computer 
Please also note that while current users of HealthlinkNY's Patient Portal will have access to their account until March 31, 2018, we anticipate that new registrations will no longer be accepted after February 1, 2018. If you already have an existing HealthlinkNY Patient Portal account you may access your record here until March 31. 2018.

Yes. Once you are logged into your account; click the "Data" tab on the left-hand side, then click "Download".
