Patient-Centered Care: With a Click, Refuah Coordinates Care

April 27th, 2016 - How Refuah Health Center found a way to operate its Patient-Centered Medical Home more efficiently.

Coordinating patient care just got easier for Refuah Health Center, a Federally Qualified Health Center with offices serving the Hudson Valley. Refuah (the Hebrew word for “healing”) is a certified Patient-Centered Medical Home (PCMH) that focuses on coordinating all care for its patients. Now, Refuah is using the HealthlinkNY Web Portal to access patient records through the Health Information Exchange (HIE). With patient permission, it can look up test results, evaluations by specialists, and even hospital discharge reports so it can manage patient health better.

“The HealthlinkNY Web Portal will be a great asset for Refuah’s Patient-Centered Medical Home teams, allowing them to view their patients’ complete medical history by simply logging into the portal,” says Christina Galanis, president and CEO of HealthlinkNY. “When patients are referred to specialists, radiologists, or any other provider outside of Refuah, team members will be able to log on see results without ever having to contact the outside organization.”

As a PCMH, Refuah tracks and coordinates tests, referrals, and transitions when patients move from one setting to another (i.e., from a hospital to a rehabilitation facility). PCMHs coordinators make appointments for patients, as well as ensure they arrive and are treated as scheduled. Afterwards, the PCMH team contacts the outside organization and relays the results to patients, so patients do not have to obtain the results themselves. The ability to go online to access information from other providers in a secure, HIPAA-compliant environment, streamlines a process that can take hours, with multiple phone calls and faxes, to complete.

Ms. Galanis says Refuah will streamline the process further when it goes live with a bi-directional HealthlinkNY connection that connects Refuah’s electronic health records (EHR) to the HIE. “This will make coordinating care for patients seamless because all the information needed will be sent directly to Refuah’s EHR,” she explains. “When team members open a patient’s record, results from outside providers will already be there.”


Operating since 1992, Refuah has medical offices in Rockland and Sullivan Counties. In addition to treating patients regardless of their ability to pay, Refuah is also a Performing Provider System (PPS) under New York State’s Delivery System Reform incentive Payment (DSRIP) Program. It’s the only PPS in New York State led by a Federally Qualified Health Center, and it includes partners that provide high quality care across Rockland and Orange Counties.
