HealthlinkNY Hires PHIP Prevention Agenda Coordinator

March 30th, 2015 - HealthlinkNY is excited to announce that Rebecca Cosgrove will be our Prevention Agenda Coordinator for the Hudson Valley working on the Population Health Improvement Plan

As the Prevention Agenda Coordinator for the Hudson Valley Rebecca will provide regional convening, capacity building and data services in support of the New York State Department of Health’s Population Health Improvement Program (PHIP). Through PHIP, eleven contractors across New York State, including THINC (now HealthlinkNY), will work to promote the Triple Aim of better care, better population health and lower health care costs. Rebecca will provide coordination and oversight of the implementation of PHIP in the Hudson Valley and coordinate with other staff on program execution. Rebecca’s primary focus will be the coordination of population health technical assistance activities in support of local health departments, hospitals and health care consumers; coordination of organization-wide consumer engagement strategies; and support of organization-wide program evaluation efforts.

Rebecca has a Master's Degree in Public Administration from Marist College and is a non-profit management professional with experience in innovative program development, promotion and oversight. She was most recently a Program Manager with Ability Beyond, where she developed and implemented all aspects of new programs for individuals with disabilities and built new community partnerships to provide enrichment opportunities for participants.  She also co-chaired the Education Committee for annual Commission on Accreditation of Rehabilitation Facilities review.  In her prior role at Ability Beyond as a Workforce Development Supervisor, she was responsible for all aspects of professional development benefits and initiatives, and career and educational counseling.

Rebecca also has volunteer experience leading trips to Haiti to build homes, where she coordinated all aspects of travel, work, communication and finances.
