It’s Free to Participate with HealthlinkNY!

There is no cost to accessing records in HealthlinkNY when you are a participating provider. Not only do you get access to patient data in your community, you can access records throughout New York State. That’s because HealthlinkNY connects you to the State Health Information Network of New York (SHIN-NY)—all at no cost.

To get the most value out of HealthlinkNY, provider organizations are encouraged to connect their EHRs to our HIE. This allows you to access HealthlinkNY directly from your EHR and download records into your system.

HealthlinkNY has pre-built connections (hubs) for many of the most widely used EHR systems. If you use one of these EHRs, you can fully connect to HealthlinkNY at no cost. We’re continually building new hubs. Call us to find out how we can connect you!

Incentives offered by NYS/CMS are available through the New York eHealth Collaborative (NYeC)!

New York State, with the support of Centers for Medicare and Medicaid Services, may provide an incentive of up to $13,000 for providers that meet its eligibility requirements. These incentives are administered by the New York eHealth Collaborative (NYeC).

Data Exchange Incentive Program (DEIP)

The New York State Department of Health (DOH), with support from Centers for Medicare & Medicaid Services (CMS), has established the Data Exchange Incentive Program (DEIP) to increase HIE adoption across the State for Medicaid providers.

Building an EHR interface connection to HealthlinkNY will increase the quantity of data in the Statewide Health Information Network for New York (SHIN-NY) and build value for the providers and patients at the point of care. Participating organizations are incentivized to contribute a pre-defined set of data elements to the SHIN-NY through HealthlinkNY. This program is designed to help defray the cost for an organization when connecting to HealthlinkNY.

New York eHealth Collaborative is coordinating the rollout of the program and the incentive payments on behalf of the State Department of Health. Limited funding is available, and this program is operated on a first-come, first served basis.

SHIN-NY Initiatives Program (SCI)

The SHIN-NY Initiatives Program is a part of New York’s ongoing work to transform healthcare delivery and shift towards value-based care. Focused on small and medium practices who are struggling to navigate the changing landscape, New York eHealth Collaborative’s (NYeC) SHIN-NY Initiatives Program practice transformation services help practices deliver high-quality, coordinated care, earn payment incentives, and prepare to thrive under value-based payment arrangements.

Modeled after national programs such as Patient-Centered Medical Home (PCMH), practices enrolled in the SHIN-NY Initiatives Program will receive up to two years of free technical assistance and support from NYeC to prepare for value-based payment arrangements.

Practices with sites that provide primary care services including internal medicine, family, and pediatrics practices are eligible to participate in APC. Practices currently receiving federally-funded transformation technical assistance (e.g. TCPI, DSRIP-supported PCMH) are not eligible to participate.

Advanced credit is available for practices who have completed federally-funded transformation (e.g. TCPI, DSRIP-supported PCMH, MU).

NYeC will support practices in Western New York, Westchester County, Rockland County, New York City, and Long Island. Space is limited and practices will be supported on a first-come, first served basis. Sign up today to ensure your spot.

New York State Patient-Centered Medical Home (NYS PCMH)

New York State Patient-Centered Medical Home (NYS PCMH) is a collaboration between the New York State Department of Health (NYSDOH) and the National Committee for Quality Assurance (NCQA) to develop an exclusive PCMH model for New York primary care practices as part of New York’s ongoing work to transform healthcare delivery and shift towards value-based care.

Soon, the NYS PCMH program will be the only path for a New York primary care practice that wants to participate in the PCMH model. The NYS PCMH model of patient-centered care sets the stage for various other reform initiatives as it aligns with the Triple Aim of improving patient care, improving population health, and reducing healthcare costs. Various payers in New York State offer incentive payments to providers who meet PCMH criteria.

All practices in New York that are interested in PCMH recognition or renewal will now follow the pathways outlined by the NYS PCMH model. Focused on small and medium-size practices who are struggling to navigate the changing landscape, NYeC’s NYS PCMH services help practices deliver high-quality, coordinated care, earn payment incentives, prepare to thrive under value-based payment arrangements, and achieve New York State Patient-Centered Medical Home recognition.
