Development of the Population Health Improvement Program (PHIP) Grows in the Southern Tier and Hudson Valley

HealthlinkNY coordinates initial PHIP Kick-Off meetings and welcomes another valuable member to our team
The Population Health Improvement Program (PHIP) continues to develop in both of HealthlinkNY’s Eastern and Western regions. HealthlinkNY hosted the initial meetings for both the Hudson Valley and the Southern Tier’s PHIP projects at the end of April. HealthlinkNY has also added a new member to its PHIP team- Adam Hughes is the new Population Health Improvement Program Manager for the Southern Tier.
In the Hudson Valley, a committee consisting of 35 regional leaders from hospitals, county Health Departments, and health centers came together to lay the groundwork for adopting a charter to implement the PHIP work plan. Bi-monthly meetings will be set to address pressing issues impacting the Hudson Valley region.
Specific workgroups will be formed around policy agenda topics surrounding community health. Taconic Professional Resources, another recognized leader in practice management, will partner with Geisinger Health System to implement a pilot program for the Patient Centered Medical Neighborhood. This program will align acute care with skilled nursing facilities, home healthcare and physician practices to insure a continuum of care for patients.
A robust communications program will engage the community and allow for much needed collaboration across healthcare divisions to improve the Hudson Valley healthcare experience overall.
The Southern Tier PHIP committee met with Rural Health Network (RHN) subcontractors, County Public Health Directors, and various other stakeholders to outline a work plan and discuss a framework for moving forward. The next steering committee meeting for the Southern Tier will be held on June 22nd, 2015.
PHIP Coordinators will be working with RHNs and the HealthlinkNY Analytics team to coordinate the gathering and analyzing of public data to support the comprehensive health needs assessment phase, which will help to identify health disparities throughout the region. RHN coordinators are also identifying existing coalitions to connect with and include in the assessment phase of the PHIP.
As the regional PHIP programs continue to develop, we welcome Adam Hughes to HealthlinkNY’s Western office. Adam is the Population Health Improvement Program (PHIP) Manager for the Southern Tier, leading the PHIP effort for these five counties. Adam’s primary focus is to ensure deliverables are met on time, provision technical assistance services are met and coordinate program evaluation efforts are completed across the five county region.
Adam has a range of public health experience in both urban and rural settings. Adam holds a Bachelor’s of Science in Human Services from Springfield College and, in addition, has earned a Master’s of Public Health concentrating in Health Policy and Management from Boston University. HealthlinkNY is excited to welcome Adam to our team and look forward to working with him.
